How To Submit Claim

MAKING A CLAIM

1. Please notify the Claims Department immediately of any possible claims.

2. You can contact your agent for the necessary requirements or you can obtain the information and claim form at this website. Alternatively, please call our Customer Line at 03 -  21160228 and our Customer Service Officers will be pleased to advise you on the Claim requirements and appropriate forms to complete.

3. You can also visit our nearest Branches. Please refer to Prudential's  list of Branches Addresses.

4. Complete the required forms.

5. Submit the completed forms and supporting documents to us via your agents or you may also submit the documents directly to us at the following address:-

CLAIM STATUS ENQUIRIES
